Insurance Application for Vendors & Exhibitors

Instructions on How to Apply

To be a participant at Vallejo Juneteenth, all exhibitors and vendors must purchase one-day insurance coverage or provide a Certificate of Insurance. Confirmation of insurance coverage must provide coverage for the day of the event and be received no later than May 15, 2025. See details in the Vendors & Exhibitors section of the Juneteenth application.

IMPORTANT: PLEASE COMPLY WITH THE REQUIREMENTS BELOW:


If you are providing a Certificate of Insurance, please contact your insurance agent or risk manager and add one-day liability coverage to your existing policy in the amounts shown below in A with the additional insureds and Certificate Holder as shown in B. DO NOT CHOOSE A LOWER AMOUNT OF COVERAGE.


If you are purchasing insurance coverage through the AAFRC's broker, you must provide the following:

A. A Certificate of Insurance for the day of the event (June 21, 2025) showing liability coverage of at least $1,000,000 (one million dollars) general liability and $2,000,000 (two million dollars) general aggregate. DO NOT CHOOSE A LOWER AMOUNT OF COVERAGE.


B. A complete copy of the additional endorsement page naming additional insureds as follows: the City of Vallejo, their directors, employees and/or agents;  NOTE – The Certificate Holder is the City of Vallejo, 555 Santa Clara Street, Vallejo, CA 94590


Also provide a complete copy of the additional endorsement page naming additional insureds as follows: the African American Family Reunion  Committee, their directors, officers, and/or agents;  NOTE – The Certificate Holder is the African American Family Reunion  Committee, 164 Robles Way, #399, Vallejo, CA 94591.


IMPORTANT: Please carefully read the Excluded Activities and Ineligible Vendor types before you attempt to purchase a policy through AAFRC.


When you receive your Certificate of Insurance, please email it to Angela Jones at qtalentcom@comcast.net
